Turn on new client forms and send them

One link collects owner & dog details, vaccinations, vet contact and a signature on your policies before their first appointment.

New client forms and online booking are separate switches with separate links. You can run either on its own, but sending the form when someone first enquires is what saves you the doorstep paperwork.

Turn new client forms on

As with your booking page, the link is built from your business name and claimed the first time you switch the feature on.

Go to Settings → New client forms.

Switch on Enable new client forms, starting the Pro trial if you haven’t already.

Copy your form link, yoursalon.groomlog.app/new, and keep it in your phone’s saved replies.

Choose what you ask for

Owner name, phone and their dogs’ names and breeds are always on the form. Under On the form you decide which of the rest you need.

Vaccination status
Yes / no / not sure per dog. Most insurers expect you to have asked.
Vet contact details
Practice name and phone number, on file before you ever need it.
Business policies
Your terms and cancellation policy, agreed and recorded with the client.
Custom text
A note at the foot of the web form: house rules, parking, what to bring. For example, “Please arrive 5 minutes early on your first visit.”

Send it the moment someone enquires

The form works best as your standard first reply. Clients open it in any browser, nothing to download, add as many dogs as they own, and sign your policies all from their phone.

Where the answers end up

A completed form arrives on your Home screen. Review it, and the owner becomes a client with their dogs, breeds, vaccination answers, vet and signed policies already attached. Nothing to type up.

From there they behave like any other client: book them in, add photos after a groom, and their notes build up visit by visit.

Someone filling in the form twice creates two submissions rather than updating the first. Accept the newer one and delete the duplicate client so their history stays in one place.
